Area Care Coordinator

My client are a well-renowned home care health provider based in the LE4 area of Leicester, who are looking for an Area Care Coordinator to join their team on a full time, permanent basis. You will be working 40 hours per week on a rota basis (Monday - Sunday) with on call responsibilities (also on a rota basis). Pay will be between £24k and £27k depending on experience and qualifications.


You will assist with the day to day running of the organisation and provide a community liaison service between the office and the community, as well as managing the staff rotas, and working alongside staff in the community. 


Service Users are based across the Leicestershire area and some Northamptonshire (mainly Coalville, Barrow, Quorn and Kettering) and although you will be office based approximately 2 days per week, the other 3 will be in the field therefore you must be a driver with access to your own vehicle. 


Duties of the role will include, but are not limited to:

  • Shadowing carers
  • Carrying out spot checks in the community and ensuring quality of care
  • Undertaking all duties as outlined in each Service User's care plan
  • Maintaining good communication and developing good working relationships
  • Carrying out personal and caring tasks that are not the responsibility of a Nurse but which could normally be expected to be undertaken by a member of the family
  • Encouraging and motivating Service Users to maintain their independence within their limitations
  • Reporting to the Registered Manager on the condition of the Service User particularly any significant change in the Service User's behaviour, needs or circumstances
  • Contacting doctors, emergency services or making appointments as required
  • Participating in the Departmental Code of Practice in relation to the administration of medication
  • Preparing and cooking meals in the Service User's home as required
  • Assisting with the budgeting of household accounts, collecting pensions, shopping and paying bills when this cannot be undertaken by others
  • Assisting in the cleaning of an exceptionally dirty home when required
  • Undertaking domestic tasks to maintain a good quality of life which the Service User cannot carry out
  • Completing all administrative documents accurately 
  • Attending meetings and reporting if necessary on the condition of Service Users
  • Dealing with conflicts and providing resolutions
  • Dealing with safeguarding/concerns/complaints/general issues


To be considered you must:

  • Be qualified to Level 2 as a minimum and be working towards/willing to work towards Level 3
  • Have a good understanding of social care and have supported living and NHS experience
  • Be able to understand and cope with emergency situations
  • Have experience of rota coordination/management
  • Be an excellent communicator
  • Have an enhanced DBS on the update system or be willing to have one run before role commencement 
  • Be a driver with your own vehicle
